Renewed Purpose works with hospitals, Community Services Boards, behavioral health providers, healthcare professionals, justice and reentry partners, community organizations, families, and other referral sources to help individuals connect with appropriate behavioral health services.

Our team reviews the reason for referral, current level of need, existing providers and supports, and the services being requested so that the next step is as clear and coordinated as possible.

We understand that referrals often occur during important transitions in care. Our goal is to communicate clearly, respond thoughtfully, and support appropriate connections between individuals, referral sources, providers, and community resources.

Supporting Continuity of Care During Transitions

Renewed Purpose welcomes appropriate referrals involving transitions from psychiatric hospitalization, crisis services, community-based treatment, and other levels of behavioral health care. When appropriate and authorized, our team works with referral sources and existing providers to support a coordinated handoff and reduce gaps between services. Our focus is not simply receiving a referral. It is helping create a coordinated path toward the appropriate next step.

Referral coordination may include:

Reviewing the individual’s current behavioral health needs.
Understanding existing treatment and support services.
Coordinating with referral sources and providers when authorized.
Clarifying the service being requested.
Identifying appropriate next steps based on clinical need.
Supporting transitions between levels of care.
Connecting individuals with appropriate community resources.

A Clear Path from Referral to Next Step

The referral process is designed to make communication and coordination as straightforward as possible.

Submit the Referral

Provide the available referral information and documentation through the appropriate service-specific pathway.

Clinical and Service Review

Our team reviews the requested service, current needs, available information, and applicable service requirements.

Additional Information When Needed

We may follow up with the referral source or appropriate contact when additional information is needed to understand the referral or determine the appropriate next step.

Review Applicable Requirements

Eligibility, medical necessity, service availability, payer requirements, authorization requirements, and other applicable criteria are reviewed as appropriate.

Guidance on the Next Step

The referral source and/or individual receives information regarding the appropriate next step based on the outcome of the review.

Important Note: Submitting a referral does not guarantee admission to a particular service. Final service placement depends on clinical need, eligibility, program availability, and applicable authorization requirements.

Community Stabilization Referral Form

The Community Stabilization referral pathway is separate from referrals for Outpatient Therapy, Mental Health Intensive Outpatient Program (MH-IOP), Peer Recovery Services, and Mobile Crisis Response.

The Community Stabilization referral form is designed for appropriate referral sources involved in discharge planning and care coordination. The information requested supports review of the individual’s circumstances, reason for referral, and transition needs.

Protecting Referral and Member Information

Community Stabilization referrals may contain Medicaid information and protected behavioral health information. Completed referrals should therefore be submitted through Renewed Purpose Treatment Center’s approved secure workflow for protected health information. The Community Stabilization referral form should not function as a standard website contact form. Completed referrals should be delivered only through the approved secure process to authorized RPTC staff.

Community Stabilization (S9482) - Use the dedicated secure Community Stabilization referral form.

Mobile Crisis Response (MCR) - Do not submit an RPTC referral. MCR is initiated through Virginia’s 988 crisis system.

Outpatient Therapy - Continue to use the live online appointment-booking pathway.

Mental Health Intensive Outpatient Program (MH-IOP) - Use the assessment/intake pathway rather than direct self-booking into treatment sessions.

Peer Recovery Services - Use the appropriate Peer Recovery Services inquiry/referral pathway rather than the Community Stabilization form.
